New Patient Forms
If you are a new patient to our office, please complete the following forms prior to your first office visit. Reviewing and filling them out before you arrive will allow us to attend to your dental needs more quickly. Feel free to call our office if you have any questions. We are happy to help!
- New Patient Registration Form: Please fill in and submit one Registration
Form per family. - Privacy Practices (HIPAA) and Financial Agreement: Please review these forms. You will sign them in our office on your first visit.
- Dental Records Release Form Cascade: Please print this form and give it to your previous dentist for us to receive records (if less than a year old).
- Medical History: Please fill out one Medical History below per family member.
